Gabapentin as a potential treatment for cough syncope.

چکیده
Chronic cough, defined as cough lasting for 8 weeks, is one of the most common symptoms of medical importance, and the prevalence in the community is 9–33%.1 A subgroup of patients suffer from the debilitating and distressing disorder of cough syncope, which is difficult to effectively manage and treat. The approach in clinic is to determine an underlying etiology for cough syncope and to treat the underlying cause with the hope that cough syncopal episodes will improve. In a situation in which cough syncope is idiopathic, current treatments are addressed directly at suppressing the sensory cough reflex. These include codeine, dextromethorphan, levodropropizine, moguisteine, local anesthetics (eg, lidocaine, mepivacaine) and low-dose morphine, yet the effects of these therapies are rather disappointing in alleviating this condition in patients. Indeed, there is a real unmet need for effective treatments in patients suffering from cough.2 It has been suggested that chronic cough is a neuropathic disorder,3 and gabapentin, a treatment for neuropathic pain, has recently shown evidence of efficacy. A recent randomized controlled trial demonstrated an improvement in cough-specific quality of life when gabapentin was used at high doses of up to 1,800 mg.4 This teaching case for health-care professionals describes the clinical assessment and medical management of a patient with cough syncope and highlights the effectiveness of low-dose gabapentin in controlling our patient’s difficult-to-manage cough.

Cough syncope induced by post nasal drip successfully managed by Gabapentin
Syncope is a common complaint in both neurology clinic and emergency department. It is defined as transient loss of consciousness with loss of postural tone, which is usually self-limited and followed by a spontaneous recovery. Our report describes a case of cough syncope resulting from chronic intractable cough caused by post nasal drip. Although his experience was debilitating, we were able t...
متن کاملA cohort description and analysis of the effect of gabapentin on idiopathic cough
UNLABELLED BACKGROUND Chronic idiopathic cough (known as cough hypersensitivity syndrome) is defined by cough in the absence of an identifiable cause. Gabapentin has been suggested as a treatment but evidence is scarce. The aim of our study was to describe the clinical features of patients with unexplained chronic cough and to investigate the effect of gabapentin (600 mg twice a day for a mi...
متن کاملMechanisms of cough syncope as evaluated by valsalva maneuver.
Successful treatment of cough syncope depends on the correction of various pathogenetic mechanisms among different patients. The valsalva maneuver (VM), which elicits hemodynamic responses mimicking coughs, has potential for investigating the individual pathogenesis of cough syncope. Eighteen consecutive patients suffering from cough-induced syncope were examined. All patients were asked to cou...

Objective. This study sought to: (1) quantify response rate and efficacy of amitriptyline, desipramine, and gabapentin in treating sensory neuropathic cough; and (2) describe an efficient treatment protocol. Study Design. This study is a retrospective case series. Methods. Persons diagnosed with sensory neuropathic cough during a one-year period were potential study candidates. To bolster the d...
